26 Revised Form – Settlement Worksheet (H-07 09/2016) - Please use the new form immediately.
This form is revised and posted as a fillable PDF form on our Forms and Instructions page.  Form revisions include additional required information. All questions must be answered. If there is any incomplete or missing information, the Settlement Worksheet will be returned and settlement approval delayed. The Commission does not accept any form filing via email/attachment or FAX.
  Pursuant to the Open Meetings Act, Annotated Code of Maryland, General Provisions Article (“GP”), § 3-302.1, the Maryland Workers’ Compensation Commission (the “Commission) is making available to the public an agenda containing known items of business to be discussed and information regarding any portion of the meeting expected to be closed. Copies of the agenda for the following meeting dates may be found on our Hearings & Meetings Schedules page. Questions may be directed to the Secretary of the Commission, Ms. Stacey L. Roig via email sroig@wcc.state.md.us.

DoIT provides Google Translate as an online tool for its users, but DoIT does not directly endorse the website or imply that it is the only solution available to users.
